II. Forms used for enterprise registration and household business registration

Starting from July 1, 2025, all enterprise and household business registration applications submitted to the competent business registration authority must use the new forms in accordance with Circular No. 68/2025/TT-BTC. 

• For enterprise registration: All documents, declarations, and papers used during the process of establishment registration, registration of changes, or other procedures related to enterprises shall follow the templates prescribed in Appendix I issued together with this Circular.
• For household business registration: The templates specifically designed for the establishment and operation registration of household businesses are detailed in Appendix II of the Circular.
